Eligible participants can receive waived frequency limitations and co-payments (D1110, D1120, D4910) when services are needed more frequently due to medical necessity as determined by the Contract Dentist.

Eligible participants can receive:
- 100% coverage for one periodontal scaling and root planning procedure, per quadrant (D4341 or D4342) every 12 months (versus 24 months)
- A combination of any 4 routine cleanings or period maintenance covered at 100% (D1110, D1120, D4346, D4910)
